New clients are expected complete the majority of office forms on our patient portal, MYIO, before the first session. Once you have decided to become a patient with Dr. Tamargo, you will receive an email invitation to access to our secure patient portal where you can complete the necessary documents prior to the first session. The forms will be provided for you at your initial evaluation. Although if preferable, you may complete any of the forms below. However, it is not required unless you are a virtual patient.
Telehealth patients will be required to sign the forms below and return to Dr. Tamargo via email or secure fax prior to the first Telehealth session. Please arrive 20 minutes before your first appointment to complete any necessary documents.
